The v-acctoken signature covers the IP that loaded the video page. Since
enrichment goes through pool proxies, every served link 403'd for clients.
Served formats now point at /proxy/rule34video/{id}/{slug}/{height}.mp4,
which reloads the page from the server's IP (1h link cache) and streams
the mp4 with range support. The pool still supplies the metadata.
The streaming response and link cache moved from the eporner proxy into
util/media_stream.rs, shared by both. USE_MEDIA_PROXY and the redirect
handler are gone.
